Here's a folded, 8.5" x 24.5" newsprint sample ballot that was put out in 1968 in a non-partisan fashion to show the three, top presidential tickets: republicans Richard Nixon and Spiro Agnew, democrats Hubert Humphrey and Edmund Muskie and independents George Wallace and Marvin Griffin. Since it mentions Thomas Eagleton for U.S. senator, this must be from Missouri, and it lists the various other locals candidates. Specific to a certain state, one of these may not have surfaced in your neck-of-the-woods. It is in nice condition but please enlarge our images to judge condition for yourself so you can be happy with your purchase.
